Hi Docs- I am attaching 3 pictures of my breasts from different angles. I'm probably a small A cup. Given the lack of breast tissue, if I came in for a consultation, what would you suggest? I ideally would like to have a large C, small D round breast that sit higher but not up to my chin. I want a natural look (silicone and under muscle only), but I do still want a teeeeeeny bit of an augmented round and teardrop shape. I am thinking 350 CCs at mod+ profile or so.. Answer: Which implant The first step is to schedule a consultation with a board certified plastic surgeon near you. During this meeting you'll learn about the procedure, discuss your goals for surgery, and have measurements taken of your breasts. These measurements are different than bra sizing. Your surgeon will combine the measurements with your stated goals to help you select a range of implants to consider. Many have sample implants available that you can "try on" to give you an idea of how you may look after surgery. I've included a video on this process that I hope you find helpful.

Answer: CC and Cup size Patients often think in terms of cup size when considering augmentation. Unfortunately, devices are sized in terms of milliliters (cc) of volume. This can lead to some confusion when sizing. Additionally, it is important to remember that cup size itself is not standardized with variations from one manufacturer to another. Unfortunately, as many women can attest their cup size in an industry leader such as VS is not necessarily transferable to another brand. Another point which is often under appreciated is that of anatomy and starting point. Any implant will add volume to the volume which is already present. The implant is additive. A particular volume will not necessarily confer the same cup size to different patients (often times it will not even confer the same cup size to different breasts in the same individual...remember they are "sisters" not "twins"). A general rule of thumb is that 125cc can represent somewhere between 1/2 to a full cup size increase. Smaller volume differentials (25-50cc) are typically less consequential representing a volume change of less than a shot glass. However, I have found these numbers, at least anecdotally, to be of little help. Patients often present with notions/goals which do not correlate with these sorts of sterile volumetric assessments. When sizing patients, there are a number of useful tools including: -3D imaging (has the added benefit of offering a volumetric analysis of the pre-operative breast) -Breast sizers (rice bags) -Goal photos I also recommend that patients commit to a particular look rather than a cup size. Once a patient settles on a look that pleases them the overall cup size increase becomes less relevant. Based on your photos and stated goals, your choice does not sound unreasonable. A smooth round, mod +/mod profile device in that size range should deliver a very pleasing result. As always, discuss your concerns with a board certified plastic surgeon (ABPS).

Answer: Moderate profile Thanks for your question! Based on what you are describing, I think you will be happiest with a moderate profile of a moderate size. I would stick with a smooth, round implant to keep youthful upper pole fullness without being too round and fake. Good luck!

Answer: Implant size It is a little more complicated than just choosing an implant size. Here is why. You have some asymmetry that would need to be corrected by lowering the left infra-mammary fold as you have a shorter distance from your nipple to the fold on the left compared to the right breast so this distance would have to be increased by lowering the left fold. It may not be possible to increase your breast to a size D based on the quality of your tissue. You would really need to be examined in order to say for sure but keep in mind that there needs to be enough stretch and laxity of your breast tissue and skin to allow for a significant change in size and sometimes in patients with very tight tissue it is not possible to change from an A to a D. In some patients that can be done without any issue and in other patients with tighter tissue it is harder to accomplish. I would lean on your plastic surgeon to help guide you in size selection as there are a lot of factors to consider in addition to the width of your breast.
