Hello and congratulations on your weight loss. I would suggest the best procedures for you are a panniculectomy and abdominoplasty with or with out lipo suction. A panniculectomy is a surgery done to remove excess skin and fat from your abdomen after weight loss surgery. It is often times covered by insurance, though abdominoplasty and lipo suction is not.

I would recommend getting an Abdominoplasty as that gets rid of any excess abdominal skin that you have as well as includes liposuction of the abdomen/mons area and tightening of the abdominal muscles (diastasis recti). All that is done in one procedure and will be most beneficial for you and your goals. Hello and congratulations on your weight loss journey! There are many options when it comes to body contouring after massive weight loss (100+ lbs). In general, for the best results we want patients to be at or as close as possible to their goal weight. Based on your photos available I would recommend a fleur-de-lis abdominoplasty to remove the excess horizontal and vertical abdominal loose skin. The trade off on this operation is accepting a vertical midline scar in addition to the low horizontal "bikini line" scar. A traditional full abdominoplasty would avoid the vertical midline scar but would likely leave some upper abdominal skin laxity in a horizontal/side-to-side dimension. You would likely also benefit from liposuction to your flanks to further enhance your contour as well as a breast lift/reduction. We are unable to see your back, but it is also common to need to address the posterior trunk in a 360 degree manner with an upper back lift/gluteal lift operation at the same time vs another surgery. This procedure removes hanging skin and excess fat from around your mid-section, on the front and back. If you are only concerned about the skin hanging below your waist (pannus) but nowhere else, you can also consider a panniculectomy.

Ideally you would undergo a fleur de lis abdominoplasty where tissue is removed along your waistline and also vertically. You can just do the lower horizontal abdominal scar if you object to the vertical one but this will not address horizontal excess.

This will allow your surgeon to tighten up your abdominal wall muscles, push down on some intra abdominal fat and remove the extra skin. Giving you the flattest belly one can give. As you continue to exercise and diet you will continue to break down more of the intra abdominal fat. Some Surgeons may also do some liposuction of the flank area understanding they are limited to what they can do at the time of a tummy tuck. Most patients will require to come back in about 3 months to contour the midsection a bit more.
