Malar bags/fluid retention and fat pad herniation. Would this require lower blepharoplasty, or is there another option? (Photos)

I’m 41 years old and have two concerns under my eyes. 1. Malar bags / fluid retention – I’m not sure if this is from poor circulation or possibly leftover filler. I had under-eye filler about 15 years ago and have tried to dissolve it, but the malar bags are still present and they fluctuate day to day. This began a couple years ago 2. Fat pad herniation – Over the past six months, I’ve noticed an inner fat pad under my eyes starting to form, though I’m not sure if this is true herniation or something else. For background, I have hyperthyroidism that has been stable for the last year (levels are normal and I’m on methimazole). Would these issues require lower blepharoplasty, or is there another surgical option?
