In this case, I think it is absolutely best to lose the weight prior to the face lift operation. Significant weight loss will not only increase the amount of skin laxity present but can unmask significant issues with bone structure and facial volume. Following weight loss your surgeon may realize an unforeseen need for facial fat injections or a facial implant (e.g. chin implant). As a facial cosmetic surgeon practicing in Manhattan and Long Island for over 20 years, I perform all types of facelifts regularly in my practice. I don’t think you need to lose all 50 pounds before you go forward with your planned facelift. However, you may want to lose some of your facial weight prior to a facelift. The purpose of a facelift is to lift sagging skin and tissue, tighten the underlying muscles of the face, and define the jawline and neck angle. If you have excess volume around the neck and jawline, you won’t get the facial definition you want from a facelift. As most of the weight you want to lose is throughout your body, and not your face, then you don’t have to lose all the weight to have a successful facelift, but losing some facial weight prior to surgery will improve your results, and overall satisfaction with the surgery. As you meet with prospective surgeons, and chosen one for your surgery, I suggest you follow up with them periodically prior to surgery as you continue to lose weight. I think the best investment is definitely surgery after you have lost the weight. A lower facelift includes lifting the cheeks, contouring the jawline, and tightening the neck. As you loss weight some of that volume loss will come out of the face and neck region resulting in anatomic changes in that area. Specifically the deflation that occurs can lead to hollowness and laxity in areas not presently apparent. These changes in soft tissue laxity and volume can be addressed during your face lift surgery. Losing significant weight after face lift surgery can result in loss of volume, recurrent laxity and in effect reverse some of what was accomplished with the surgery.
