I am 39 years old, generally slim and youthful in appearance. I am 5'5 and 124 pounds, and have two children. I gained and lost about 50 pounds in each pregnancy. My neck skin is extremely wrinkly, crepey, and saggy. My face and body look 35, but my neck looks like it belongs to a 55-year old. I KNOW I have some sun damage there from some severe burns I got in my teens. I am also wondering if the weight gains and losses of pregnancy contributed to the sagging. What can I do?

From your photo you appear to have loose skin, some residual fat and lax neck muscle. A Neck Lift with repair of the muscle through a small incision under your chin, and tightening of the muscle and removal of loose skin through incisions behind the ears should give you a very nice result.

Microneedling can be done but this has only a very mild effect, and probably not enough to make you happy. there are a couple of radiofrequency options available, but none have been found to have a drastic result. I agree that given your age, a mini neck lift with a submental incision only and tightening of the muscles and redraping of the skin may have a reasonable improvement, but you would probably be best served by a neck lift with incisions around the ears. This would give you the best result possible. The weight gain and loss during pregnancy can certainly stretch out the neck skin and cause it to look the way it does now. Based on what you are describing along with the pic, I think a short scar neck lift (scar only under chin) would be a reasonable option to consider. However such an approach needs to focus on the deep layer of the neck below the platysma muscle while preserving all the remaining fat on top of the platysma muscle below the skin. Removing any more of this fat layer will likely make the neck look worse. However, focusing on the deeper layer which consist of a deep fat pocket, large salivary glands, and bulky digastric muscles can make a significant improvement without any skin removal which is usually not necessary in the 30's.
