I had a red about 8 years ago and would like to go smaller. My goal is to narrow the breast and remove side fullness, I would like to not have to wear a bra..…so maybe a small b cup.

  Your goal to narrow the breast and go smaller can be accomplished with a revision of your breast reduction.  You look very good right now and appear to be a B cup,  but that doesn't  matter, you can go smaller if you want to.
 The nipples are pointing high and the nipple to fold distance is excessive, so as part of the revision, while reducing the horizontal width (modestly to avoid an unesthetic loss of lateral curvature), excess skin and breast tissue would be removed as a horizontal crescent at and above the inframammary fold.
   To get the result you want, close communication with your plastic surgeon is necessary so you have a good mutual understanding and agreement on what  your goals are and what you want done.
